KF2 Field Medic Tips: Weapons by Tier and Loadout Ideas (Humorous)
A community primer on the Field Medic that tries to be funny while giving sensible advice. It covers what the Medic does, its five weapon tiers, how to fight while healing, ten sample loadouts and a handful of extras, as of the SWAT update era. For more serious Medic references see KF2 Field Medic guide and Medic perk.

The guide's opening image, a joke about the wrong game.
What the Medic does
The Field Medic is the only class that can truly heal teammates; healing with another class is, as the author puts it, as useful as a 9mm against a Fleshpound. Besides healing, the Medic covers the team against small ZEDs such as Crawlers, Stalkers, Bloats and Cysts. The main job is to heal as much as possible.
Weapons
Unlike other perks, which have one weapon family (for example assault rifles for Commando), the Medic uses five different kinds of guns across five tiers.
| Tier | Weapon | Notes |
|---|---|---|
| 1 | HMTech-101 Medic Handgun | Part of the starting kit. Slightly more damage than the 9mm, 15 rounds, good fire rate, reflex sight that shows ammo, and a syringe attachment for long-range healing. |
| 2 | HMTech-201 Medic SMG | 40 rounds, less damage than the handgun, high fire rate, good recoil; same sight and a slightly buffed syringe. |
| 3 | HMTech-301 Medic Shotgun | The most powerful in terms of damage, 10 shells, auto-shotgun, similar reload speed to the handgun, can fire 3 syringes before cooldown. |
| 3 (alt) | Hemogoblin | Fires large cylinders that cause bleeding; bigger ZEDs need more hits. 8 shots, same syringe as the shotgun. Useful against Scrakes and Fleshpounds. |
| 4 | HMTech-401 Medic Assault Rifle | The most useful: 40 rounds, good rate of fire, damage and reload, lots of spare magazines. The strongest syringe: 4 darts and a low cooldown. |
| 5 | HMTech-501 Medic Assault Rifle | 30 rounds and slightly stronger shots, with an underbarrel grenade launcher that fires medic grenades exploding on impact. Fewer spare magazines and no syringe attachment, which makes squad healing hard. |
Fight and heal
- Medic weapons deal less damage than other guns but can easily kill small and medium ZEDs and add firepower against a Fleshpound, a Scrake or a boss. Always aim for the head.
- You are priority number one: a dead Medic cannot heal and the squad is likely to wipe.
- Healing earns you experience and dosh.
- You cannot save everyone, especially players who run into the horde and keep moving.
- Medic grenades do not explode with damage: they release a blue gas that heals the team and poisons ZEDs of every kind, which can save you when cornered.
Loadout ideas
Listed as primary then secondary:
| Name | Primary | Secondary |
|---|---|---|
| Basic Medic | Medic guns of tier 1 and 2 | 9mm (can take two) |
| Advanced Medic | Medic guns of tier 3 and 4 | Dual 9mm |
| Desert Medic | Medic gun of tier 2, 3 or 4 | Single Desert Eagle, 9mm |
| Cowboy Doctor | Any Medic gun, Winchester | 9mm |
| Revolver Doctor | Any Medic gun, S&W M500 | 9mm |
| SharpMedic | Medic gun of tier 3 or 4, M14 | 9mm |
| De-Mem-Man (idea by phantom7489) | HM401 rifle, M79 | 9mm, Desert Eagle or M1911 |
| SWAT Medic | Any Medic gun, any SWAT gun | 9mm |
| Military MeeM | Any Medic gun, M4 with M203 | 9mm |
These are only ideas. The author advises against carrying Berserker weapons: the melee role is the Berserker's.
Extras
- Keep friends close: easier for you to heal them and for them to cover you. Keep Husks, Scrakes, Fleshpounds and especially Sirens away, because of your grenades.
- Face the enemy directly when you can: lots of armor lets you act as a tank sometimes, though this is risky.
- The Medic is one of the fastest classes, so run when someone needs help.
- Healing earns a lot of dosh; use it for better guns but leave money for the team's equipment.
- Always buy a Kevlar vest, since the Medic's only natural defense is poison resistance. Choose armor even over a higher-tier gun.
- After the SWAT update the perk that repaired armor was removed, but new abilities such as becoming a "living medical grenade" in Zed Time were added.
- Choose your playstyle: full healer, warrior Medic or a mix, and pick skills accordingly.
FAQ
- What are the Medic's weapon tiers in this guide?
- HMTech-101 handgun (tier 1), HMTech-201 SMG (tier 2), HMTech-301 shotgun or the Hemogoblin (tier 3), HMTech-401 assault rifle (tier 4) and HMTech-501 assault rifle with an underbarrel grenade launcher (tier 5).
- What is the Hemogoblin?
- An alternative to the shotgun that fires large cylinders which cause a bleeding status. Bigger ZEDs need more hits to start bleeding, and a bleeding ZED is debuffed and loses health over time. It holds 8 shots and has the same healing syringe as the shotgun.
- Should a Medic use Berserker weapons?
- The author says no: the melee role belongs to the Berserker, and the Medic's scalpel often needs more than one hit to kill a Clot.
